Request for support
You can use this referral form for your child, for yourself if you are over 18 or if you a professional referring another individual that you are supporting. We will accept referrals for children aged 10 years and 8 months, they will be assessed as soon as possible and they will be put on the waiting list, this is so that by the time they get near the top of the list, they are 11 years of age.
This form will take 5-10 minutes to complete.
